Driveway or Sidewalk in City Right of Way (Licensed Sidewalk Builders) Permit Request

  1. Permit Review Approval Process

    Complete permit applications are reviewed and processed in the order they are received. It may take up to three (3) full business days to receive an approved permit.

    A complete permit application includes ALL of the following items: 

    1. Permit application.
    2. Contractors must have insurance and bond on file with the City’s Risk Management department.  Any questions please call 920-448-3125 or email certificates.insurance@greenbaywi.gov.
    3. Construction Schedule if work exceeds the allotted two-week timeframe.

    Work may not begin until all approved permits are received from The Department of Public Works.

  2. Do you have a Sidewalk Builders License?*

    You MUST obtain a Sidewalk Builders License to receive a Driveway Apron or Sidewalk Permit. 

  3. Is your proposed work on a principal arterial, minor arterial, or collector roadway?*

    If you're not sure, check the Roadway Classifications map to determine.

  5. Address of Proposed Work within City Right of Way
  6. Please describe exactly what work will be taking place at this location. Example: Installing driveway apron. 

  7. How many total days for the work to be completed within the timeframe you are requesting?

  8. If the time frame exceeds two weeks, a Construction Schedule is required.

  9. What are the nearest cross-street boundaries of your excavation?

    Example: Work on N Jefferson Street between Cherry Street and Walnut Street.

  10. For partial street closure, which side of the street will be obstructed?*

  11. Obstruction Information*

    Mark all areas that apply for where you will be obstructing / parking.

  12. To expedite permit review include plan with all cut locations and sizes.

  13. If you have a PDF of a traffic control plan, excavation plan, additional site plan documents, etc., you may upload them here to assist in review of request.

  14. Electronic Signature Agreement*
    By checking the "I agree" box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, 3) you may still be required to provide a traditional signature at a later date, and 4) you have read and agree to the Terms and Conditions document.
  15. END OF APPLICATION. Click "Submit" so that the Department of Public Works can review your permit request. If your permits are approved, you will receive an email notification within 3 business days with a link to download your approved permit documents.

    There are NO fees for this permit.
