Family Savings Plan
The Family Savings Plan allows you and your family the opportunity to have up to 100% coverage for eligible out-of-pocket expenses when using Network Health participating providers.
Questions specific to FSP enrollment services:
UMR
UMR is the health plan administrator for the City of Green Bay and can assist you with questions, including:
- Is my provider in-network?
- How do I read my Explanation of Benefits (EOB)?
- How do I get a new ID card?
- Is a precertification or preauthorization required for my procedure/appointment?
UW is Contracted with United Health Care Choice Plus Network
You and your family members enrolled in the City’s health plan can access University of Wisconsin Hospital and Clinics and the charges will be allowed at the in-network benefit level. If you have a dependent attending UW Madison, please let them know they have a local in-network provider.
